Permission to use AUPO mailing lists may be granted to members, organizations, firms or individuals who agree to and comply with the terms and conditions. The use of AUPO mailing lists may be approved to promote products, events and/or services of interest to AUPO’s membership and related to research, education and patient care. Advance payment is required, and all mailing pieces must be submitted for review and approval before orders are processed. Lists are provided in Excel format only. Requests for email addresses will be declined.

AUPO may help announce your event and/or service of interest to AUPO’s membership and related to research, education and patient care. Below are commonly asked questions about AUPO’s mailing list rental service. Your mailing piece must be reviewed and approved before your order will be processed.

 

Q: Do you provide email addresses?

A: No

 

Q: Do you offer the mailing list in an electronic format such as Excel?

A: Yes, AUPO’s mailing list is provided in Excel format for one-time use only.

 

Q: Will you invoice for mailing list rental services?

A: No. Payment by Visa, MasterCard, or check is required before processing or sending the mailing list.

 

Q: How soon after ordering will I receive the mailing list?

A: All orders are processed within 1–2 weeks. We try to process orders as quickly as possible; however, at certain times of the year there may be a delay.

 

Q: What is the Confidentiality Agreement for Mailing List (Licensee) form?

A: This form grants permission (a limited license) to the purchaser (licensee) to use AUPO’s list for one specific mailing. By signing this form, the Licensee agrees to the terms of AUPO’s policies and terms set forth in the agreement. All orders submitted to AUPO requesting the rental/purchase of any part of our list will need to also include a signed copy of the Confidentiality Agreement for Mailing List (Licensee) form. This form must be completed by the Chair at academic institutions or an authorized signer for the organization or individual ordering the list; it may NOT be signed by a mailing house representative.

 

Q: I have a mailing house that will do my mailing. Will I still need to sign the Confidentiality Agreement for Mail List (Licensee) form?

A: Yes. If you have a mailing house that will be preparing your mailing, AUPO will also require them to sign a mailing house confidentiality agreement form. We will contact your mailing house and send the appropriate agreement form to them for a signature. No list will be released until all forms are completed, signed, and on file.

 

Q: What is a mailing piece?

A: A mailing piece is the announcement that you wish to mail. Your mailing piece will be approved according to policies governing the rental of mailing lists.

 

Q: When will I find out if my mailing piece is approved?

A: If your mailing piece is approved, your order is processed automatically within our timeframe. If there is a problem with your mailing piece, we will contact you.

 

Q: My mailing piece is not ready yet, may I submit last year’s?

A: No, only current mailing pieces will be accepted for approval.

 

Q:  My mailing piece is not ready yet, can you start processing my order anyway?

A: No, only complete orders will be processed.

 

Q:  My mailing piece file is too large to attach to the submission form, is there another way to send it?

A: Yes, email the mailing piece separately to [email protected]. If it is too large to send as an email attachment, contact the AUPO office at [email protected] or (415) 561-8548 between 9:00 AM – 4:00 PM Pacific Time for assistance.

 

Q: How will you send my order?

A: All orders will be sent electronically in a secure format to the designated individual.

 

Q: What type of lists do you provide?

A: The lists will be provided in Excel format. Emails will not be included.

 

Q: Can I use one order form for multiple requests or multiple lists?

A: No. If you are ordering multiple and/or separate lists, you will need to provide separate order forms and agreements for each. You will also need to provide a copy of what you are mailing.

 

Q: Are your lists certified?

A: Our lists will be provided as is.

The Association of University Professors of Ophthalmology (AUPO) is comprised of approximately 650 members – Chairs, Residency Program Directors, Associate Program Directors, Research Directors, Directors of Medical Student Education, Vice Chairs of Education, Administrators, and Residency Program Coordinators – in the United States and Canada. Permission to rent AUPO’s membership mailing lists may be granted to organizations, firms or individuals who agree to and comply with the terms and conditions below.

Procedure:
You must submit the following with each order:

  • A copy of the intended mailing piece – you must submit every piece that will be included in the mailing.
  • A completed order form, including payment.
  • A completed and signed copy of the Confidentiality Agreement for Mailing List (Licensee) form. This must be completed by the Chair at academic institutions or an authorized signer for the organization or individual ordering the list, not a 3rd party mailing house.
  • Following AUPO’s approval of your mailing piece, AUPO will send you one mailing list for one-time usage. If you will use a Mailing House, AUPO requires that your Mailing House sign a copy of AUPO’s Confidentiality Agreement for Mailing Lists (Mailing House) form which will be sent to them by AUPO after we receive your complete order and are ready to process it.

Terms of Use:

Submission of an AUPO Mailing List Order Form acknowledges acceptance of AUPO's Terms of Use as follows:

  • AUPO mailing lists are available only for communications germane to the scientific or practical aspects of academic medicine.
  • AUPO mailing lists will not be made available for any communication on drugs, medical equipment or devices involving the offer of a rebate, discount, or other remuneration, which in the sole discretion of AUPO, may be illegal or unethical.
  • AUPO mailing lists will not be made available for any fundraising or survey mailing purposes.
  • AUPO mailing lists will not be made available for any communications that would tend to mislead, misinform, or deceive.
  • Mailing pieces are subject to advance approval by AUPO. The AUPO reserves the right to deny any request for mailing lists for any reason.
  • Rental of AUPO’s mailing lists does not imply AUPO endorsement, and no implication of endorsement, approval, co-sponsorship or cooperation will be made in any mailings.
  • AUPO mailing lists may not be used to create or update any database. The mailing list is the property of AUPO and may not be duplicated.
  • Each AUPO mailing list may be used only once and only for the purpose approved by AUPO.

Mailing List Pricing:

  • We do not accept Rush Orders.
  • To order mailing lists, complete the enclosed order form and payment and return it to AUPO with the intended mailing piece and the completed Confidentiality Agreement for Mailing List (Licensee) form. Once your mailing piece has been approved, a mailing list will be produced according to your specifications and emailed within 1-2 weeks. (Note: We aim to process labels as quickly as possible; however, at certain times of the year there may be delays.)
  • Orders will not be processed without payment.
